摘要: Objective To estimate the long and mid term results of total hip arthroplasty ( THA ) of Crowe type III-IV developmental dysplasia of the hip with dislocation in adults.Methods A total of 29 patients ( 32 hips ) were treated from March 2007 to December 2011. Age ranged from 22 to 57 years old ( average: 43 years ). Eighteen cases were of left hips, and 14 cases of right hips. The Harris score of involved cases was ( 41.5 ± 5.3 ). The shortened length of involved extremity was 2 - 6 cm ( mean 4.2 cm ). According to Crowe scale: type III 10 cases ( 11 hips );type IV 19 cases ( 21 hips ). Patients’ main clinical manifestations were unilateral or bilateral hip pain, claudication, and limited motion of the hip joint. Some patients had waist pain. X-ray, CT iflms indicated dislocation or complete dislocation. All operations were performed by the same doctor. All cases were followed up for 5 - 9 years ( mean: 6.5 years ).Results All incisions were healed during the ifrst intention. No thrombosis, infection, pneumonia, hip dislocation or sciatic nerve injury occurred. Femoral nerve injury occurred in 2 case and was cured after certain treatment. Pain relief of the hip was achieved, and claudication status was improved. Research at the latest post-surgery follow-up showed the mean length difference was 0.4 cm ( range: 0.3 - 1.0 cm ) between the affected and contralateral extremities. The Harris scale was ( 68.72 ± 5.83 ) 3 months after the surgery, ( 77.90 ± 4.63 ) 6 months after the surgery, ( 86.53 ± 2.65 ) 1 year after the surgery, ( 87.75 ± 1.82 ) 2 years after the surgery, ( 87.88 ± 1.59 ) 3 years after the surgery, ( 88.14 ± 1.26 ) 4 years after the surgery and was signiifcantly improved to ( 88.4 ± 1.1 ) 5 years after the surgery with statistical signiifcance (t = 6.845,P< 0.001 ). The X-ray showed stable interface between the acetabular prosthesis and the bone. No infections or dislocations occurred. No revisions were involved in the follow-up.Conclusions THA is an effective and reliable method in the treatment of Crowe type III-IV developmental dysplasia of the hip with dislocation in adults, improving patients’ status.
